Forms are provided in two different formats for your convenience. Regardless of the format, all forms require various signatures. They must be printed from your computer, signed as appropriate, and mailed to American Live Stock Insurance Company.
In most cases, your agent will provide instructions on what to do with a form after you have completed it. |

Microsoft Word format

Use this option if you want to type your responses onscreen, which should be a time-saver.
When you click on a link to a form in Word format (.DOC), your computer will open a copy of the form in Microsoft Word (you must have Word installed on your computer).
(Note: When downloading the Microsoft Word version of a form, you may be asked for a password. This may be due to a "bug" in some browsers. Simply click on "cancel" and the form should download properly.)
You should see gray rectangles in the fields where data must be entered.
Position the cursor over a gray box and begin typing. Your data should appear on screen. Use the TAB key (or the mouse) to move to the next field for entering data. (If desired, you can simply print the blank document and complete it by hand, the old-fashioned way.)
When data entry is complete, save the form under a new name. Print the document, sign it as required, and mail to American Live Stock (the mailing address is shown here). |

Adobe Acrobat format

If you don't have Microsoft Word on your computer, consider using the Adobe Acrobat (.PDF) version of our forms. (You must have Adobe Acrobat Reader, a free program available from www.adobe.com.)
When you click on a link to an Acrobat form, the Acrobat Reader will start on your computer. Merely print the form, complete it by hand, and mail back to American Live Stock. |
